What to bring:
- A current referral letter. GP referrals are valid for a year and specialists’ referrals are valid for 3 months.
- Medicare, private health insurance, DVA or pension card where applicable.
- Current medications and recent test results.
- Any recent hospital discharge summary, where applicable.
- Comfortable pair of shoes and clothing as you may need a treadmill test/stress echocardiogram.
Payments and costs:
- While making the appointment, the cost of consult will be informed. Please discuss the costs involved with our office while making the appointment.
- Full payment is needed on the day of visit.
- Debit, visa or master card payments are accepted.
Cancellation Policy
Once booked for an appointment, any cancellation is to be done prior to 24 hours unless under special circumstances to allow another patient to get the benefit of the appointment.

If experiencing a sudden onset of severe symptoms like chest pain, breathlessness, loss of consciousness or palpitations, please call 000.
